The Role of the Periodontist in Airway Focused Dentistry: Diagnosis and Surgical Treatment of Tethered Oral Tissues using CO2 Laser and Combination Technique

Tethered oral tissues (TOTs), or ties, are restrictive frena, tight connective tissues that limit the movement and function of the lips, tongue, or cheeks. These restrictions can impair feeding, growth and development, breathing, sleeping, speech and posture in children and adults. Diagnosis requires a comprehensive approach, and treatment may include therapy and surgical treatment such as Frenectomies or Frenuloplasties.

Milton Geivelis, DDS, MS
Dr. Geivelis is a Periodontal Surgeon, whose practice is limited to Periodontics, Dental Implants, and Laser Surgery. Dr. Geivelis is in private practice in Bartlett, IL. He is also on staff at Northwestern Medicine (Central DuPage Hospital), Department of Surgery.

Born in Athens, Greece, Dr. Geivelis received his dental and graduate training from Northwestern University. He has a Doctor of Dental Surgery Degree, a Master of Science Degree, and a Specialty Degree in Periodontics and Dental Implants. Dr. Geivelis is a Diplomate of the American Board of Periodontology. From 1993 to 2001, Dr. Geivelis was professor and director of Graduate Periodontics at Northwestern University Dental School, training the residents in Periodontal Surgery and Dental Implants. He has been involved in academic research and has authored and co-authored several scientific articles. His clinical interests have been in Soft Tissue Grafting, Bone Regeneration procedures, and Computer Guided Surgery for the placement of Dental Implants. In the last 12 years however, Dr. Geivelis has dedicated a significant amount of his time to Airway issues and Sleep. He has performed 4000+ LFF procedures in children and adults and 8000+ Frenectomies in infants. Dr. Geivelis lectures on Periodontics, Implants, as well as Tethered Oral Tissues nationally and internationally.

Laser Therapy in Periodontal and Peri-Implant Therapy: Understanding Nd:YAG and Er:YAG wavelengths and their use for the Periodontist

For too long, periodontists have pushed aside the use of light energy to treat periodontal issues. Two wavelengths Nd:YAG and Er:YAG can make treatment of various periodontal and peri-implant diseases much more predictable. Using lasers as a tool can change the way you practice.

Learning Objectives:

  • To understand Nd:YAG and Er:YAG laser wavelengths.
  • Identifying the target tissues for each laser wavelength.
  • Understanding the benefits of using lasers vs. conventional methods
  • Treating periodontal issues and periodontal disease
  • Treating peri-implant disease
  • Treating and removing soft-tissue lesions
  • Reviewing how light energy can help healing (photobiomodulation)
  • Hands-on use of both wavelengths with pig jaws, tissue, teeth, and
    implants

Alan Dalessandro, DDS
Dr. Alan Dalessandro graduated with honors from Loyola University School of Dentistry in 1979.

He then spent several years at Rush Presbyterian/St. Luke’s Medical Center in Chicago doing a general practice residency, general anesthesia, facial and reconstructive plastic surgery. He completed his periodontal residency at the University of Illinois Dental School in 1982.  He is in full time private practice in Hoffman Estates, Illinois.  Dr. Dalessandro teaches laser use in clinical periodontics to dental residents and residents in the Army and Navy.  He also teaches solo and group practices, and he formally teaches as a trainer for a laser company both in the US and Canada.

Bone Grafting to Optimize Surgical Outcomes for the Periodontist

This presentation will begin with a review of the key elements of success for the numerous bone grafting challenges facing the periodontist.  Bone grafting for implant site development will be demonstrated from simple to complex defects with ideal treatment planning, site development, guided surgery and provisionalization.  Peri-implantitis treatment with bone grafting to repair failing implants will be described along with thoughts on prevention and maintenance.  While implants offer an attractive solution to the failed dentition, the goal of the periodontist is still to save teeth.  Bone grafting for periodontal defects around teeth will be described for buccal and interproximal bone defects to improve the prognosis.

  • Review the key differences in how autogenous bone, cellular bone allografts, PDGF, BMP-2, autologous blood products, enamel bone matrix proteins and amniotic derived products optimizebone grafting outcomes.
  • Describe cellular bone allograft protocols for different applications of this unique bone grafting material.
  • Review bone grafting strategies for treating peri-implantitis and periodontal disease.

Bradley S. McAllister, PhD
Dr. McAllister is in private practice in the Portland Oregon area.

He completed his residency in Periodontics and PhD at the University of Texas Health Science Center at San Antonio after obtaining his DDS at the University of Washington.  Dr. McAllister has lectured on implantology and periodontics to national and international audiences. His interest has focused on innovative regenerative therapies.  He has published numerous research articles in professional journals. He is a member of several professional organizations and is very active in teaching.

45 Years of Soft Tissue Reconstruction: What Have We Learned?

During the past four decades we have seen significant advances in the field of soft tissue grafting around teeth and implants. Unfortunately, many of these techniques are based on empirical concepts and outcome data, not on sound anatomical principles. Recent findings have shed new light on our understanding of  anatomy relevant to soft tissue grafting. In addition, our improved understanding  of the  role of phenotype, tooth position and tooth movement has shed new light on the progression and management of root exposure. Finally, the increasing periodontal problems seen with the expanding use of clear aligner therapy throughout dentistry cannot be ignored.

With the help of an internationally recognized anatomist,  we will begin with a detailed discussion of new anatomical findings relevant to soft tissue grafting.  Then, drawing on a combined 90 years of clinical experience, we will explore the latest principles relevant to soft tissue reconstruction.  Attendees will come away with a clear understanding of the role of phenotype and tooth position on the initiation and progression of soft tissue deformities, as well as the development of a sound diagnostic and treatment protocol for the prevention and management of soft tissue deformities around teeth and implants.

  • Identify the periodontal anatomy that can affect the long term clinical results of mucogingival surgery

  • Analyze the importance of phenotype, tooth position and location in root exposure

  • Understand the role of clear aligner therapy in the development of mucogingival deformities

  • Evaluate the role of biologic modifiers in periodontal plastic surgery

  • Develop a comprehensive model for diagnosing and treating mucogingival deformities

Joe Iwanaga, DDS, PhD
Department of Neurosurgery, Tulane Center for Clinical Neurosciences, Tulane University School of Medicine

Dr. Joe Iwanaga, DDS, PhD is an oral and maxillofacial surgeon and clinical anatomist currently serving as Professor and Clinical Anatomy Fellowship Director in the Department of Neurosurgery at Tulane University School of Medicine. He earned his DDS from Tokyo Medical and Dental University and his PhD in Medicine from Kurume University. With over 700 peer-reviewed publications, Dr. Iwanaga is internationally recognized for his contributions to head and neck anatomy and for advancing the integration of anatomical science into surgical practice. He is a contributing author to Gray’s Anatomy, including the chapter on the mouth, and serves as the Dental, Oral, and Maxillofacial Section Editor of Clinical Anatomy. He is an International Advisory Board Member for Netter’s Atlas of Anatomy (7th and 9th editions). Dr. Iwanaga has held visiting professorships in Japan, Thailand, and the Caribbean, reflecting his strong commitment to global anatomical education. In addition to his academic work, he directs multiple private dental anatomy and surgical training courses worldwide, providing hands-on education for general dentists grounded in contemporary anatomical science. His achievements have been recognized with numerous honors, including the 2025 IAOMS/IJOMS Research Publication Award and multiple awards from the American Association of Clinical Anatomists, underscoring his leadership in clinical anatomy research and education.

Peter O. Cabrera, DDS
Marquette University School of Dentistry | Private Practice, Chicago, IL

A native of Havana, Cuba, Dr. Cabrera completed his undergraduate studies at Northwestern University, earned his DDS from the University of Illinois, and his certificate in periodontics from Northwestern University Dental School. In addition to his private practice in Chicago, Dr. Cabrera served on the affiliate staff of Lurie Children’s Hospital for 35 years. He has lectured nationally and internationally at institutions including the University of Illinois, Case Western Reserve University, the University of Missouri–Kansas City, Illinois Masonic Hospital GPR, Marquette University, and the University of Barcelona in Spain. Throughout his career, Dr. Cabrera has been deeply engaged in organized dentistry, having served as President of both the Illinois Society of Periodontists and the Midwest Society of Periodontology. His lifelong clinical focus has been soft tissue reconstruction. He is the lead author of recent publications in the International Journal of Periodontics and Restorative Dentistry on the periodontal manifestations of Wire Syndrome and a landmark 37-year retrospective on connective tissue grafting — the longest follow-up of its kind published to date.

Timoth P. Walsh, DDS
Private Practice, Westmont, IL

Dr. Timothy Walsh earned his BA in Biology from St. Mary’s University in Winona, Minnesota in 1975, his DDS from Loyola University School of Dentistry in 1979, and completed his residency in Periodontics at the University of Illinois in 1981. Dr. Walsh has held faculty positions at both Loyola University School of Dentistry and the University of Illinois College of Dentistry. He has been deeply engaged in organized dentistry throughout his career, having served as President of the Illinois Society of Periodontists (1992) and President of the Midwest Society of Periodontology (2005). He served as District 4 Trustee for the American Academy of Periodontology from 2012 to 2018, and has served as Executive Director of the Midwest Society of Periodontology since 2018.

He is currently in private practice in Periodontics and Dental Implants in Berwyn and Westmont, Illinois.
