Minimally Invasive Hard and Soft Tissue Management in the Anterior Esthetic Zone: From Tooth Extraction to Implant Placement
Presented by: Shaun Rotenberg, DMD, MS
The need for hard and soft tissue augmentation in preparation for implant placement and restoration is determined at the time of tooth extraction. The more severe the defect, the more hard and soft tissue reconstruction will be needed to create an esthetic result, especially in the anterior. Techniques in guided bone regeneration (GBR) and soft tissue augmentation have improved significantly over the years. Our ability to augment bone is now more predictable and successful; however, traditional techniques utilized during socket reconstruction can actually have a negative effect on the final esthetic result. These include papilla loss, loss of keratinized tissue, and apical migration of the gingival margin. The purpose of this course is to educate clinicians on how to predictably manage and enhance the soft tissue from extraction all the way to final restoration in even the. most severe defects.
Learning Objectives:
- Identify the periodontal anatomy that is being preserved during extraction and socket reconstruction to achieve an ideal esthetic result
- Perform multiple techniques and utilize novel materials, both allograft and xenograft, to preserve and enhance the natural anatomy at the time of extraction
- Understand the rationale for technique selection based on the situation presented
- Discuss proper treatment timelines based on technique selection
- Use described techniques and materials to improve the health and esthetics of dental implants at the time of placement
- Hands-on with pig jaws
- Handling, preparation, and use of Cytoplast MicrodermTM, ZdermTM, and CytoplastTM TXT-200 dPTFE membrane
- Practice advanced minimally invasive recipient site techniques utilizing a vestibular access
- Practice recipient site suturing techniques with various suture materials
Dr. Shaun Rotenberg is a board-certified periodontist and an assistant professor at Ohio State University. He is a diplomate of the American Board of Periodontology and has authored/co-authored several peer-reviewed articles in journals such as the Journal of Periodontology and The International Journal of Oral & Maxillofacial Implants. Dr. Rotenberg’s current research interests include soft tissue augmentation and peri-implant disease, and he has presented at both national and international professional meetings.

Contemporary Approaches for Managing Periodontal and Peri-implant Diseases
Presented by: Paul S. Rosen, DMD, MS, FACDEvolutionary developments in grafting materials, biologics, and treatment algorithms are enabling clinicians to save diseased teeth and failing den-tal implants more predictably than in the past. Clinicians are faced with a myriad of solutions, but what is the best approach(s)? Should the failing tooth be saved? If the implant with advanced disease is lost, are there any options available? This lecture will focus on the principles behind our evolution in regenerative medicine and will try to provide some of the bio-logic basis for both the materials and algorithms that we select.
Learning objectives:
- List the currently available grafts, biologics, and barrier materials and techniques involved in regeneration which can help increase predicta-bility.
- Discuss the role of lasers in the management of periodontitis and peri-implantitis.
- List some of the historic principles of wound healing needed to achieve successful regenerative outcomes.
Dr. Paul Rosen maintains full-time private practices in both Yardley, Pennsylvania and New York, New York limited to periodontics, surgical implant placement and regenerative therapy. He also has appointments as a Clinical Professor of Periodontics at both Rutgers University School of Dental Medicine and the University of Maryland Dental School along with being Adjunct Professor at New York University College of Dentistry. Dr. Rosen is a Diplomate of the American Board of Periodontology and has served in the past as an Examiner for the Board as well. Dr. Rosen is a reviewer for and serves on the editorial advisory boards for a number of dental journals. He has authored/co-authored a number of articles and several book chapters on periodontal regeneration, implants and interrelated orthodontic-periodontal care and lectured both nationally and internationally on the subjects of dental implants and regeneration for both teeth and dental implants. Dr. Rosen is honored for his work by receiving a number of prestigious awards including the American Academy of Periodontology’s Master Clinician Award in 2015, the Northeastern Society of Periodontists Isidor Hirschfeld Award for Clinical Excellence in 2016, and the Distinguished Alumnus Award for Professional Achievement from the University of Maryland Dental School in 2020.

Minimally Invasive Therapy in Periodontics: The Art and Science of Saving Teeth
Presented by: Diego Velásquez-Plata, DDS, MSDMinimally invasive therapy in the health sciences has been extensively documented in the literature since its introduction by Fitzpatrick and Wickham in the early 90’s. The field of periodontics has not remained foreign to these concepts and their applications: preparing radicular surfaces with confidence, tracing precise incisions, executing flap designs with minimal extensions, delivering biomaterials accurately and achieving passive wound edge approximation, are all part of a carefully choreographed therapeutic approach that ultimately benefits the patient. Less morbidity and higher probabilities of success in maintaining natural teeth are premeditated consequences derived from this novel approach which has been facilitated and optimized by the incorporation of the operating microscope and the microsurgical principles and philosophies supporting this modus operandi.
This presentation will showcase the rationale behind minimally invasive procedures utilizing a microsurgical approach in periodontal therapy by illustrating clinical cases in a wide scope of applications pertaining to the daily workflow in periodontal treatment.
Learning objectives:
Those attending this presentation will:
- Review the historic background in regenerative periodontal therapy, minimally invasive procedures, and microsurgery.
- Understand the physiological events that take place and the anatomical features that frame regenerative healing in periodontal therapy.
- Identify the components of the microsurgical approach.
- Adopt mechanisms to implement post-operative protocols for the short- and long-term success of regenerative periodontal therapy.
- Differentiate the financial benefits of maintaining natural teeth in comparison to their replacement.

Diego Velásquez-Plata, DDS, MSD is a graduate of the Pontificia Universidad Javeriana School of Dentistry, Colombia. He was awarded an Ambassadorial Scholarship by The Rotary Foundation that allowed him to complete his postdoctoral training in Prosthodontics and Dental Materials at Indiana University School of Dentistry where he also received a Masters degree in Science of Dentistry. He went on to complete a residency in Periodontics at The University of Texas Health Science Center at San Antonio.
Dr. Velásquez-Plata works in private practice in Fenton, Michigan and is an Adjunct Clinical Assistant Professor at the School of Dentistry of The University of Michigan.
He is a Diplomate of the American Board of Periodontology and is currently an Associate Editor of Clinical Advances in Periodontics, a member of the peer review panel of the Journal of Periodontology, and a consultant of The International Journal of Periodontics and Restorative Dentistry. He is part of the Clinical Advisory Team of The Osteology Foundation, has published a number of articles on prosthodontics, periodontics, microsurgery, and dental implant related topics and has lectured nationally and internationally. He is the co-editor of the book titled “Microsurgery in Periodontal and Implant Dentistry – Concepts and Applications”.
Dr. Velásquez-Plata is a recipient of the prestigious “Dr. and Mrs. Gerald M. Kramer Scholarship for Excellence” award of the American Academy of Periodontology Foundation. His private practice-based research activity has been recognized by the American Academy of Periodontology by receiving The Clinical Research Award in 2017 and 2021. He is past president of the Midwest Society of Periodontology, Michigan Periodontal Association and the American Academy of Periodontology Foundation.
